Once an online application has been completed, the following information will need to be sent to FACTS to complete the application process:
- Copies of your most recent Federal tax forms including all supporting tax schedules.
- Copies of your W-2 forms for both you and your spouse.
- Copies of supporting documentation for Social Security Income, Welfare, Child Support, Food Stamps, Workers’ Compensation, and TANF.
All supporting documentation can be uploaded in PDF format online. Documentation may also be faxed to 1-866-315-9264. If you need additional assistance, the business office can help in uploading your tax documents.
One Application – Note Special Deadline
Your FACTS assessment will also be used to determine your family’s eligibility for Archdiocese High School Tuition Assistance, so only one application is necessary. To qualify for Roncalli Catholic and Archdiocesan assistance, your application and documentation must be submitted and verified by FACTS no later than Feb. 28. Any concerns about this deadline can be directed to Roncalli Catholic’s Business Office.
